主要理念：
本計畫以創建「青銀共創、世代融合之智慧高齡友善社區」為願景，從促進人才培育與協助城鄉教育發展著手，並強化區域產學鏈結，擴大協助服務大台南之高齡社區，推動世代融合之高齡友善社區，成就在地老化之樂活環境。除了新增與其他公民團體（台灣高齡照護暨教育協會）及吉安醫院合作，進行社區長輩健康醫療服務評估，並擴大結合跨校系及異業結合之跨專業領域，以提高社區高齡者之健康關懷外，更將依健康醫療服務評估結果與資料之分析，協助安排後續之社區健康照護計畫，以提高高齡者之健康與生活福祉；實踐教育部鼓勵大專校院與區域連結合作，教授走入社會與社區進而實踐大學社會責任，達到協助社區永續經營的目的。
The vision of project aims to create smart and age-friendly communities by youth and senior cooperation.
It starts with promoting the cultivation of talents and assisting urban and rural education development, and to strengthen regional links between industry and education, and to serve the elderly community in Tainan. Promote age-friendly community of generational integration and achieve an active environment for aging in place. We cooperation with other university of science and technology and the citizens' associations , such as Taiwan Senior Care and Education Association and Ji'an Hospital to be a teamwork. We will conduct assessments of community health medical services for elders, and inter-industry linkages to improve the health of elderly in the community. In addition to health care, it will also assist in arranging follow-up community health care plans and improve the health and well-being of senior in accordance with the results of medical and health assessment. Encourage teachers and students to go out of school. Practice university social responsibility and achieve the goal of assisting the community in sustainable management.
計畫簡介：
青銀共創之高齡友善社區營造；產學攜手促進高齡樂活之在地發展；促進智慧健康照護及高齡福祉服務人才培育。以期能呼應全球積極共同推展之確保社區居民健康生活及促進各年齡層的福祉，促使社區(城市)與人類居住具包容、安全、韌性及永續性，與促進和平且包容的社會等多項目標，以提升未來與國際社會實踐對接之可能性。
The main three goals of the project are age-friendly community empowerment by youth and senior cooperation, industry-academies collaborating to promote the development of elder lives, promoting intelligent health care and talent educating for the well-being services of elderly. The aims of project is in order to respond to global initiatives to ensure the healthy life of community residents and promote the well-being of all ages. Make the community (city) sustainable and promote a peaceful and inclusive society. And enhance the possibility of linking the future with the practice of international society.

解決方案：
(一) 高齡社區，健康照護問題
1. 本計畫將基礎健康醫療評估導入社區，結合各專業醫療團隊及各跨校系之師生，發揮專長提供社區中65歲以上長者周全性老年評估及醫療諮詢
2. 找出需高關懷之個案，除了協助轉介至里長及醫療服務端外，更可作為未來社區對長者後續相關健康照護與長照服務協助與介入的參考依據
3. 產學攜手，共同落實智慧健康照護服務之實踐
4. 藉由PBL課程，導入高齡智慧照護之跨域專業整合課程，與長者共同研發生活照護輔具與創新應用，提高社區長者之生活品質
5. 協助在地失業居民完成照服員培訓，以協助在地就業，解決在地照護人力之問題
(二) 高齡友善社區建置
1. 結合在地學子實施小小照服員訓練，從小即養成其親老、敬老之觀念與習慣，主動關懷在地長者
2. 與在地幼兒園/國中小學校結合，共同推動世代融合活動
3. 居家環境安全評估
4. 產業攜手共創居家安全服務網絡平台
5. 設置一美食地圖app
