We focus on the elderly friendly community to create, and industry cooperation to train talent, promote friendly cities for the "respect for the elderly, pro-old, unobstructed, smooth travel, housing, connectivity, health, not old", and actively create a suitable environment for the elderly life, support the elders healthy aging, living and aging friendly community to do deep cultivation.
計畫簡介：
本團隊致力推動青銀共創之高齡友善社區營造；透過產學攜手促進高齡樂活之在地發展；連結跨專業能量促進智慧健康照護及高齡福祉服務人才培育。計畫透過「青銀共創、世代融合之智慧高齡友善社區」為願景， 強化區域產學鏈結，成就在地老化之樂活環境。進行居家安全評估與世代融合之活動，並結合跨校系及異業結合之跨專業領域，提高社區高齡者之健康關懷，更將執行經驗分享至在地公民團體，以協助安排後續之健康照護規畫，提高高齡者之健康與生活福祉；實踐教育部鼓勵學校走入社會與社區進而實踐大學社會責任，達到協助社區永續經營的目的。
We focus on promoting the " Creating Smart and Age-friendly Communities: Youth and Senior Cooperation " a Goal , enhance the environment of happy living, home safety assessment and friendly activities, and combine multi-professional cooperation, improve the health care of the elderly in the community, improve the health and well-being of the elderly, practice university social responsibility, to assist the community sustainable operation purposes.

欲解決問題：
本團隊實踐場域面臨問題主要之服務面向:（一）高齡者居家安全與健康照護等問題（二）社區照顧人力不足，透過居家評估後導入居安服務，舒緩照護人力問題；促進社區高齡者樂活之高齡友善社區。以作為未來居家安全服務之進行更精緻化的改善與建議，使之能更易於被高齡者接受且可廣為推廣等。
The problems faced by the team on the implementation, (1) the elderly family safety and health care and other issues (2) community care manpower shortage, through home assessment after the introduction of residential security services, to ease the care of manpower problems, promote the elderly friendly community, and enhance the quality of safety services, so that the elderly accept and use.
解決方案：
(一) 高齡社區健康照護問題1.導入社區健康醫療評估，提供高齡者老年評估及醫療諮詢，找出需高關懷個案，協助轉介醫療服務端，更作為未來參考依據2.產學攜手，落實智慧健康照護服務之實踐3.藉由PBL課程，導入高齡智慧照護之跨域課程，與長者共同研發創新生活照護輔具，提高其生活品質。(二) 高齡友善社區建置(照顧資源)1.結合在地學子實施小小照服員訓練，從小養成親老、敬老之觀念與習慣，主動關懷在地長者2.與在地幼兒園/國中小學校結合，共同推動世代融合活動3.居家環境安全評估4.產業攜手共創居家安全服務網絡平台與手機app。
For (i)health care for the aged, solutions include implementing 1.community
health assessment and 2.smart health care services, also to 3.co-develop daily
living aids with the aged; whereas for (ii)building an age-friendly community
(care resources), solutions include 1.training local pupils in care skills,
2. promoting inter-generational interaction from schools; 3.performing home
environment hazard assessment; and 4.building a home safety service network
未來願景：
期待在本團隊與高齡社區透過「圓桌共聚」模式之共同商討與互動模式下，啟動青年創意與創業之動機，並激勵銀髮再造之活力，依社區需求，並與產學緊密結合，共同推動並促進台灣銀髮族相關產業商機或文化特色等之發展 ，如與長者共同研發生活輔具與創新應用，創造建置社區在地安老支持系統，均以社區高齡者需求來發想，含社區醫療照護地圖、社區美食及安全食品商家地圖等 ......
攜手在地，放眼國際，共築高齡友善社區達永續。
The team strives toward a self-sustaining age-friendly community, by joining hands with the local community - involving community seniors via “round table gathering” mode to spark creativity and entrepreneurship in the younger generation while revitalizing same in the aged to meet the community’s needs along with industry partners to promote senior- or cultural-related industries and business opportunities – while setting sights on international horizons.
